Sports Physical Forms: 2025-2026
2026 Boys Basketball & Winter Cheer:
Tryouts for boys basketball and winter cheer will start on Tuesday, January 20th, 2026 from 3:15-4:45pm. Boys basketball players should meet in the main gym, all cheerleaders should meet in the cafeteria. All athletes should be prepared to attend tryouts for that entire week. All athletes from boys basketball and winter cheer should have a ride home at 4:45pm. There are no buses.
Informational meeting for Boys Basketball will take place on TBA in RA (2:25pm) in the main gym.
Informational meeting for Winter Cheer will take place on TBA in RA (2:25pm) in the cafeteria.
All three participation requirements or the winter re-certification form must be completed online by Thursday, January 15th, 2026 for ALL boys basketball players and winter cheerleaders.
Looking for the WINTER RE-CERTIFICATION FORM? Log into your ATS Account, click the tab "forms" and select "Winter Re-certification form" from the drop down menu.
Athletes will NOT be able to participate in tryouts without all three participation requirements or the winter re-certification form. No exceptions!

PIAA Physical Forms
ALL 5 PIAA forms must be filled out on the ATS Online System.
Please do NOT hand any paperwork to a coach, teacher, athletic director, counselor, nurse or the front office. Please also do NOT mail, drop off, fax or email any paperwork to the school or athletic director. These will all NOT be accepted.
All PIAA forms are good for one school year.
We do NOT accept the paper form of the PIAA physical packet. Please print out section 6 of the PIAA physical packet (link below) and bring it to your doctor. They will sign and date section 6, and then you can scan it (or take a picture with your phone) onto the ATS website under the tab "e-files". Please make sure it is signed and dated after May 1st 1st, 2025 in the bottom right-hand corner or it will NOT be accepted.
While on ATS, you will also need to e-sign/agree to several other terms and conditions. All 5 forms on ATS must be filled out, and section 6 must be uploaded before your child is cleared and able to participate in tryouts.

2025-2026 PIAA Section 6 Physical Form
* Remember: Paper copies are NOT accepted ! Use the link above ONLY to download section 6 for your doctor. Upload it to the ATS system under the tab "e-files" once it has been completed by your doctor. Please do NOT email, drop off, fax or mail this form into the school. It will NOT be accepted. Please make sure Form 6 (athletic physical) is signed and dated after May 1st, 2025 at the bottom of the form. It will NOT be accepted unless it is signed and dated in the bottom right hand corner.

* NEW for the 2025-2026 school year: There is now a 2 step authentication the first time you log into your ATS account. You will need to access your child's CRSD email for the verification email in order to log into your ATS account. You can access your child's email through GMAIL if needed. If your child does NOT remember their password to their CRSD email, please keep reading:
- Students who have set up password recovery via ClassLink can reset their own passwords. Go to www.crsd.org/classlink and click the "Help, I forgot my password" link and follow the prompts. You will need to enter your Student ID number. If recovery has been setup, you will verify your identity via your security questions or by receiving a text message at your registered phone number. Complete the process to reset your password.
- Students can also enter an IT Helpdesk ticket at crsdstudent.sherpadesk.com. Complete the necessary fields, and be prepared to provide phone numbers and another email address where you can be reached! Follow ups will occur through those email addresses and/or phone numbers.
** If you are a returning athlete and you have used the ATS system before, your OLD password will NOT work. ALL passwords from last year have been re-set to the default password listed above. After you log into ATS for the first time this school year, you will be asked to change your password.

The GENERAL tab is for information about the STUDENT ATHLETE. The emergency tab is where the parent information should be.
HOW TO FILL OUT THE FORMS ON THE ATS SYSTEM:
To fill out the physical packet click on the “Forms” tab. There are 5 forms that need to be completed and submitted.
PIAA SECTION 2: CERTIFICATION OF PARENT/GUARDIAN
PIAA SECTION 3: UNDERSTANDING OF RISK OF CONCUSSION AND TRAUMATIC BRAIN INJURY
PIAA SECTION 4: UNDERSTANDING OF SUDDEN CARDIAC ARREST SYMPTOMS AND WARNING SIGNS
PIAA SECTION 5: HEALTH HISTORY
PIAA CIPPE FORM 6: Under the “eFiles” tab you will find PIAA Section 6. Please print it out from the link above and take it to your physician.
The PIAA and CRSD cannot allow any student to participate in ANY ACTIVITY until a valid, current physical is completed and on file. You can scan section 6 (or take a picture with your phone) onto the ATS website.
We DO NOT ACCEPT any other Physical Form other than the Section 6 – PIAA CIPPE Form that is located under the “efiles” tab. Pennsylvania Department of Health School Physical Forms are NOT ACCEPTED.
PLEASE ENSURE ALL PHYSICAL FORMS ARE SIGNED DATED ON THE BOTTOM AFTER MAY 1st, 2025 BY THE MEDICAL PROVIDER.
Please DO NOT upload your child's immunizations to this site. The document will be deleted. Immunization records must be given to the school nurse if needed.
Re-Certification Form: Winter and Spring Sports
Looking for the WINTER RE-CERTIFICATION FORM??? Log into your ATS Account, click the tab "forms" and select "Winter Re-certification form" from the dropdown menu. DO NOT FORGET TO PRESS NEW!
Fill out the Re-certification form for Winter sports on the ATS online system under "forms". Fill out this form if you have already played or attended tryouts for a sport during the fall.
