The Council Rock School District requires a physician's written order and parent/guardian authorization for a specific medical procedure to be done in school. Download and submit this form to comply.

Permission Form: Student Medication

Prescribed medications which are necessary for the health of a child may be administered during the school day. Download and submit this form to comply with CRSD guidelines.

Permission Form: Self-Administered Medications

CRSD requires written parental consent for the authorization of student handling of self-administered medical devices, including inhalers. Download and submit this form to comply.
